• All Staff and professionals are fully vaccinated

• Staff and patients are required to wear masks covering  the mouth and nose (no neck gaiters or bandanas)  the entire time they are inside the building. Therefore, eating/drinking must be done outside (or we must sanitize offices after eating/drinking inside). If you do not have a mask, please call our office.

• Family members/friends may not enter the waiting room (please call the office if you have any special needs).

• Please sanitize or wash hands before entering the office, and wash hands frequently.

• Our staff members are sanitizing surfaces and door knobs between patients.

• Please allow staff to take your touch-free temperature before entering.

• Patients are welcome to call the office at (205)329-7815 for more information. Please reschedule visits by calling the office if anyone is not comfortable with our attempts to keep the office as safe as we can during these difficult times.

1) You have had any Covid-19 symptoms, such as fever, cough, new onset shortness of breath, or flu-like symptoms.

2) You had contact with someone that had Covid-19.
